چکیده
We aimed to identify clinical factors associated with recurrent infective endocarditis (IE) episodes. The characteristics of 2816 consecutive patients definite IE (January 2008–2018) were compared according the development a second episode IE. A total 2152 out 2282 (94.3%) patients, who discharged alive and followed-up for at least first year, presented single IE, whereas 130 (5.7%) recurrence; 70 cases (53.8%) due other microorganisms (reinfection), 60 (46.2%) same microorganism causing episode. Thirty-eight (29.2%), whose recurrence was microorganism, diagnosed during 6 months follow-up considered relapses. Relapses nosocomial (OR: 2.67 (95% CI: 1.37–5.29)), enterococci 3.01 1.51–6.01)), persistent bacteremia 2.37 1.05–5.36)), surgical treatment 0.23 (0.1–0.53)). On hand, episodes reinfection more common in chronic liver disease 3.1 1.65–5.83)) prosthetic 1.71 1.04–2.82)). relapse appear be different. better understanding these would allow effective therapeutic strategies.

Repeat infective endocarditis: differentiating relapse from reinfection.
Repeat infective endocarditis due to the same species can represent relapse of the initial infection or a new infection. We used time-based clinical criteria and pulsed-field gel electrophoresis-based molecular criteria to classify 13 cases of repeat infective endocarditis as either relapse or reinfection. The agreement between clinical and molecular criteria was imperfect (agreement in 10 [77%...

Background: Sub-acute bacterial endocarditis (SBE) rarely presents with features of a small vessel vasculitis. Patients with SBE can also develop multiple serological abnormalities including ANCA. In this report, we present a case of infective endocarditis mimicked ANCA associated glomerulonephritis.Case presentation: A 57-year old male with a clinical picture of rapidly progressive renal failu...

BACKGROUND We sought to provide a contemporary picture of the presentation, etiology, and outcome of infective endocarditis (IE) in a large patient cohort from multiple locations worldwide. METHODS Prospective cohort study of 2781 adults with definite IE who were admitted to 58 hospitals in 25 countries from June 1, 2000, through September 1, 2005. RESULTS The median age of the cohort was 5...

Sir—The much-needed trial by Dow et al. [1] regarding treatment duration for urinary tract infection (UTI) in patients with spinal cord injury and the accompanying editorial commentary [2] raise questions regarding the determination of whether isolates from recurrent (posttherapy) episodes of UTI represent relapse or reinfection and determination of the possible sources of such infections.
